What is a dermal roller?

The Derma Roller is an amazing device widely used for micro needling to increases the levels of collagen and elastin in your skin. The derma needling has been used successfully to treat fine lines, wrinkles, lax and sun-damaged skin to reduce the appearance of stretch marks; improve acne ice pick scars; and induce hair’s regrowth. It has also been used to tighten skin after liposuction. Dermarolling can be safely performed on all skin types and ethnicities.

How does dermarolling work?

In skin needling, needles are used to puncture the skin to create a controlled skin injury. Each puncture creates a channel that triggers the body to fill these microscopic wounds by producing new collagen and elastin. Through the process of neovascularization and neocollagenesis, there is improvement in skin texture and firmness, as well as reduction in scars, pore size, and stretch marks.

a. Wound healing Mechanism/Collagen Induction Therapy (CIT) 

Collagen Induction Therapy (CIT) or Percutaneous Collagen Induction (PCI) therapy is a regenerative method used to improve skin structure. Very fine needle lesions stimulate the skin to renew itself, without any significant damage to the epidermal tissues. As a consequence, needling greatly reduces the risk of complications and side effects compared to ablative and semi-ablative skin procedures. These fine incisions can extend over up to 50% of the skin surface without causing scars. The proviso is that the individual lesions are no bigger than 0.3mm in diameter. Clinical and scientific data underpin the efficacy of the method in the treatment of wrinkles and lines, stretch marks and various types of scars.

b. Microneedle Mesotherapy

The dermarolling creates thousands of microchannels deep into the epidermal layer which greatly encourage the transepidermal transport of topical active substances. In practical terms, Needle lengths of 0.1–0.3mm are normally used in Cosmetic Needling for mesotherapy purpose. The needle puncture the superficial layers of the stratum corneum to improve the penetration of topically applied active substances. Since the needle-pricks remain in the epidermis, there is no intradermal bleeding and injuries.

We can distinguish between the following needling techniques as regards the intended physiological and clinical effects of the treatment

Cosmetic Needling (0.1–0.3mm needles)

This type of needling does not cause percutaneous collagen induction [PCI] . It is for mesotherapy purpose to enhance penetration of topically applied active ingredients. It is a purely cosmetic treatmentHowever, in some cases, it may stimulate desquamation and improve the surface quality of the skin. Basically, if active ingredients are not used topically, then there will be no significant change to the skin.

Medical Needling (0.5–2mm needles, at this depth one can expect PCI)

In most cases, the percutaneous collagen induction (PCI) method only includes needling treatments with needles from 1mm in length, which reach the dermis and produce at least a minimal intradermal hemorrhage.

Surgical Needling (2.5-3mm needles, PCI)

Surgical Needling with 2.5~3mm needles, which reach from the reticular dermis to the subcutis, leading to heavier intradermal bleeding. The longer the needles used, the more extensive is the potential post-interventional collagen induction and the regenerative effect on the skin. Moreover, there is also an increase in the pain associated with the treatment, as well as the postoperative edema and bruising.

Long needles 3mm in length are used for more severe clinical conditions, mostly involving medical indications, such as conspicuous and hypertrophic scars (e.g. burn scars, ice-pick acne scars). Scar tissue hypertrophy requires deep penetration into the dermis and heavy intra-dermal bleeding if the desired remodeling of the papillary and reticular dermis and the associated significant improvement in skin quality is to be achieved. The surgical needling is mainly performed by professionals in clinics.

What's the right needle length for my skin condition?

The needle length is selected based on skin thickness and treatment applications. The actual skin thickness varies among different people. As skin respond very differently with different needle depth, it's important to choose the right length according to your skin thickness and desired applications.
If the patient wants skin rejuvenating therapy, the degree of skin aging and the wrinkle type are the key criteria for selecting the correct method or, more specifically, the most suitable needling technique. For this condition, needle length should be over 1mm. If patients only need to enhance active ingredients absorption, 0.1~0.3mm should be ideal.
